THIS IS WORKING AGAIN- a few problems getting the site to load last night but it's working now!

Hi! I'm having trouble completing the form. I always get ot the second page and it says please complete form before submitting, but I already completed it. It already happened 3 times! Please help!

When I tried I got a message saying that they are not recruiting in my area (Dallas, TX.) There were links to other sites for coupons/discounts.
